AI Audit Report Template

Adapt an original filled AI audit report specimen with scope, procedures, findings, limitations, management responses and evidence references.

This AI audit report template provides an original filled specimen and practical instructions for replacing its contents with evidence from a real engagement. It is intended for audit leads, technical assessors and organisations commissioning an assessment. The specimen concerns a hypothetical procurement tender-addendum summariser. It is not a report about a real customer, a certification, a legal opinion on tender obligations or evidence that OpenForAI completed the illustrated work.

Use the specimen as a drafting aid, then remove every illustrative fact that does not belong to the engagement. The separate AI audit report structure guide explains the reasoning architecture. This page focuses on usable fields, example wording, evidence references and release checks. A template cannot supply missing evidence or authorise a conclusion that the work did not support.

How to use this specimen

Replace the system, period, versions, criteria, procedures, observations and decisions with verified engagement information. Keep the distinction between observed facts, interpretation, management response and limitations. Do not retain the example's sample counts or finding identifiers as decorative detail. If a field cannot be completed, identify whether it is not applicable, unknown or unavailable and explain the effect on the report.

The specimen uses internal criteria approved for a hypothetical assessment. They are not presented as regulatory clauses or protected professional-standard text. GAO's Government Auditing Standards are a public reference for professional audit work in their applicable setting; this original template does not claim conformance with them or make them binding on a Saudi commercial technical engagement. Determine the actual reporting requirements separately.

In a real report, also record the assessed period, fieldwork completion, evidence cut-off and issue date. These dates have different meanings. Do not use the issue date as though it were the period examined. If a corrected version is observed after fieldwork, describe that later development separately and state whether it was included in the assessment.

Template section 2: executive decision summary

Illustrative report wording: The assessment examined whether the identified pilot workflow preserved selected material changes from approved tender addenda and linked summary statements to the applicable source revision. The work used 12 purposefully selected tender packs: 4 Arabic, 4 English and 4 containing mixed-language material. The selection was designed to examine specified conditions and does not support a population error-rate estimate.

Illustrative report wording: Two distinct control issues were identified in the selected evidence. One summary omitted a documented extension of a submission deadline. A separate summary linked a changed requirement to an earlier source revision. The assessment did not establish that an external submission was made on the basis of either summary, and external submission controls were outside scope. Management should decide the permitted pilot conditions and require suitable correction evidence before considering expansion.

Illustrative report wording: The available evidence supports findings about the tested configuration and selected cases. It does not establish the effectiveness of every production deployment, legal compliance of the underlying tender process or universal correctness of generated summaries. Management's planned changes are recorded separately from verified remediation.

Adaptation instruction: Replace the entire summary with the actual results. Keep material limitations beside the conclusion. Do not state that all controls are effective when the detailed report contains untested objectives. Do not imply an actual financial loss, missed tender or legal breach unless the engagement has appropriate evidence and authority for that conclusion.

Template section 3: scope and criteria

Filled illustrative scope and criteria record
ItemSpecimen content
Included useGeneration of draft addendum summaries for procurement staff review within the controlled pilot.
Included componentsSelected source ingestion, retrieval, generated draft and evidence of the review handoff.
Excluded activitiesExternal tender submission, legal interpretation of tender conditions and supplier award decisions.
Criterion TA-C1Internally agreed criterion: selected material changes identified by the authorised source owner must remain visible in the draft or be explicitly referred for review.
Criterion TA-C2Internally agreed criterion: a cited source must identify the applicable addendum revision for the statement it supports.
Criterion authorityHypothetical procurement process owner approved the criteria for the assessment; no external regulatory status is claimed.
Known access limitAvailable pilot records support selected-case inspection; production-wide outcome evidence was not obtained.

Adaptation instruction: Replace internal criteria with the actual agreed requirements and identify their source and applicability. A requirement can be contractual, internal, regulatory or a proposed assessment criterion; preserve that distinction. Use the scope agreement to reconcile boundaries before issuing the report. Where applicability is unresolved, disclose the question rather than inventing a legal conclusion.

Template section 4: methods and evidence

Filled illustrative procedure matrix
ProcedureEvidence and selectionWhat it can establishWhat it cannot establish
TA-P1: source authority checkInspect approval and revision references for the 12 selected packs.Whether the selected source references match the assessed criteria. The legal adequacy of every tender document.
TA-P2: material-change comparisonCompare approved source changes with preserved drafts through qualified bilingual review.Observed preservation or omission in the selected outputs.Population prevalence from purposeful selection.
TA-P3: citation revision checkTrace cited passages and revision identifiers for selected statements.Whether those references support the corresponding statements. That every future citation will be correct.
TA-P4: review-handoff inspectionInspect available pilot referral and review records. What the selected records show about the handoff.Unobserved external submission or award outcomes.

Illustrative report wording: The case selection intentionally included changed dates, revised requirements and mixed-language material. The review used a documented rubric and preserved disagreements for adjudication. The sample size and selection reflect this diagnostic purpose. No statistical confidence interval or overall population accuracy claim is reported.

Adaptation instruction: State the actual selection mechanism, population, denominators and adjudication method. Distinguish unique cases from repeated runs and unresolved outcomes from passes. Link calculations and source comparisons to controlled working papers. A report table should summarise the procedure, not replace the underlying evidence needed to reconstruct it.

Template section 5: finding specimen A

Filled original finding TA-F1: omitted material change
FieldSpecimen entry
CriterionTA-C1 requires the identified material change to remain visible or be explicitly referred.
ConditionOne selected draft omitted the source owner's documented extension of a submission deadline.
EvidenceObservation TA-O1, source revision reference TA-E2 and preserved draft TA-E3, linked through workpaper TA-W1.
ScopeOne purposefully selected case under the identified pilot configuration; wider frequency was not estimated.
ConsequenceA reviewer relying only on the draft could work from an incomplete account of the addendum. Actual external use was not established.
CauseUnresolved: retrieval selection and summary generation require separate diagnostic examination.
Required responseMaintain authoritative-source review while diagnosing and correcting the omission pathway.
Closure basisAgreed original and new cases meet TA-C1, with relevant deployment and review evidence.

Adaptation instruction: Replace the source and draft references with controlled evidence identifiers. The finding should state what was observed before attributing cause. If the actual assessment supports a different consequence, explain its evidence. Do not retain the specimen's wording merely because it sounds serious. A severity label should be supported by the engagement's severity rationale, not copied from another report.

Template section 6: finding specimen B

Filled original finding TA-F2: source revision mismatch
FieldSpecimen entry
CriterionTA-C2 requires the citation to identify the applicable revision for the supported statement.
ConditionA separate selected summary linked a revised requirement to an earlier addendum revision.
EvidenceObservation TA-O2, revision comparison TA-E4 and workpaper TA-W2.
ConsequenceThe reviewer could be directed to a source that does not substantiate the current statement, weakening verification.
Cause statusIndex update timing is a hypothesis; the assessment has not established it as the cause.
Required responseInvestigate revision selection and make the applicable source relationship verifiable.
VerificationTest new revision-change cases and inspect the relevant source-index configuration and deployment scope.

Adaptation instruction: Keep this issue separate from TA-F1 because the criterion and verification question differ. If real observations share a control weakness, explain the grouping rather than copying the specimen's number of findings. Record confirmed and suspected wider exposure separately. Use the findings register to preserve relationships between observations, findings, actions and decisions.

Template section 7: limitations and unresolved matters

Illustrative report wording: Selection was purposeful and covered the identified pilot cases rather than the full operational population. The assessment therefore reports observed exceptions without estimating their prevalence. External submission records and production-wide outcomes were not examined. No conclusion is expressed about whether a tender response was submitted, accepted or rejected on the basis of an AI summary.

Illustrative report wording: The identified configuration and preserved source revisions define the technical boundary. Later changes are not covered unless separately described and tested. Where causal explanations remain hypotheses, the report identifies diagnostic work rather than presenting those explanations as established facts. The report does not evaluate the substantive legal meaning of the tender requirements.

Adaptation instruction: Every material limitation should name the missing evidence or excluded procedure and the conclusion it prevents. Avoid a generic disclaimer that leaves the reader guessing. If a limitation undermines the requested decision, bring it into the executive summary. If alternative procedures partly address it, state both their contribution and the remaining gap.

Template section 8: management response and follow-up

Illustrative response plan, not a record of completed work
FindingManagement responseAccountable roleVerification needed
TA-F1Retain source review, investigate omission and implement an approved correction.Procurement process owner, with technical action owners.Original and unseen variation tests, deployment evidence and review operation.
TA-F2Investigate revision mapping and correct the source-reference pathway.Source-index owner, with procurement review.Revision-change tests and evidence that the correction reaches the affected scope.
Both findingsProvide an authorised decision on pilot conditions pending verification.Designated sponsor or risk decision owner.Decision rationale, scope, conditions and review trigger.

Adaptation instruction: In a real report, include agreed target dates, dependencies, interim controls and management's actual response. Do not invent commitments for an owner who has not made them. If management disagrees, record the disputed element and the assessor's evidence-based response. A planned correction is not a verified correction; follow the retest process before using that disposition.

Template section 9: bounded conclusion

Illustrative report wording: For the identified pilot configuration and selected cases, the evidence showed departures from TA-C1 and TA-C2. The assessment therefore does not support an unqualified conclusion that the draft-summary workflow consistently met those criteria. The report identifies the corrective and evidential work needed to inform management's expansion decision. It does not grant deployment approval or certify the system.

Adaptation instruction: Write the conclusion after reconciling findings, methods and limits. If the evidence supports a favourable bounded conclusion, say what it covers. If it is insufficient, say what was not established. Do not force every engagement into the specimen's outcome. The wording must follow the actual work, not a desired commercial result.

Template section 10: evidence and release record

Illustrative evidence index for report traceability
ReferenceDescriptionPurpose
TA-E1Selected-case manifest and selection rationaleSupports the method and coverage boundary.
TA-E2 and TA-E3Controlled source and draft pair for TA-O1Supports the omission observation.
TA-E4Revision comparison for TA-O2Supports the source-reference mismatch.
TA-W1 and TA-W2Procedure, adjudication and reviewer recordsConnect observations to findings and limitations.

Adaptation instruction: Keep sensitive attachments in an authorised repository and provide controlled references rather than embedding unnecessary personal or commercial data. Record report review, factual corrections, approval, issue version and authorised distribution. A report that cannot be linked to its supporting evidence set is difficult to defend when challenged or revised later.

Template section 11: completed release-review specimen

The following original record shows how a report can be checked before issue. It is a completion aid for the report itself, not another assessment of the AI system. Replace the illustrative entries with the actual review performed, preserve unresolved points and identify the person authorised to release the report. A blank approval field should not be replaced by an invented signature or professional credential.

Illustrative report release-control record
Review questionSpecimen review entryRequired disposition
Do all sections describe the same scope? The title, summary and conclusion refer to the pilot draft-summary workflow; external submission remains excluded.Retain the boundary in any approved extract.
Do sample counts reconcile? The method and summary both identify 12 purposeful packs, with 4 Arabic, 4 English and 4 mixed-language packs.Do not convert these counts into an operational accuracy claim.
Are findings traceable?TA-F1 links to TA-O1 and TA-W1; TA-F2 links to TA-O2 and TA-W2.Confirm authorised access to the final supporting evidence set.
Are causes distinguished from hypotheses?Retrieval selection and index timing remain proposed explanations requiring diagnosis.Preserve the uncertainty in the issued wording.
Are management commitments genuine? The specimen records proposed responses only; no real owner commitment exists.Replace with actual authorised responses in a real engagement.
Does the conclusion imply certification? The specimen expressly limits the conclusion to the tested criteria and cases.Remove any unsupported badge, approval or certification claim.

Illustrative reviewer note: The draft conclusion initially described the workflow as suitable for unrestricted use, but the evidence only addressed selected pilot cases and identified unresolved findings. The conclusion was revised to describe the departures and the work needed to inform expansion. This change concerns the support for the conclusion, not a stylistic preference. The final review record should identify the actual question, response and amendment rather than merely stating “reviewed”.

Illustrative distribution wording: This report is intended to inform the identified sponsor's decision within the stated scope. Any authorised extract should retain the relevant criteria, period, findings and material limitations. The report must not be represented as a certificate or as evidence that untested systems, versions or business outcomes were assessed. Sensitive supporting material remains subject to its separate access conditions.

Adaptation instruction: Agree distribution language with the actual engagement owner and relevant advisers. Do not use this specimen to impose contractual terms that were never agreed. If a third party needs reliance on the report, resolve that requirement explicitly before sharing. A document created for one internal decision may not answer a different buyer's procurement or regulatory question.

When a report is corrected after issue, identify the superseded version, changed content, reason and recipients requiring the update. A later remediation result can be a separate follow-up report instead of rewriting the historical finding. Preserve the distinction between correcting an error in the report and reporting a change in the system after the assessment. The release record should make that distinction visible.

Before release, search the adapted document for every specimen identifier and illustrative count. Confirm that any retained value belongs to the actual engagement and has supporting evidence. This simple editing check prevents a reusable template from introducing invented facts into an otherwise sound report.

For a scoped AI audit, bring the reporting decision and a masked example of the evidence available. The template can then be adapted to a real, agreed engagement rather than treated as a ready-made opinion. Discuss the report and evidence requirements for your AI audit.

FAQ

Your questions, answered.

Can I copy this AI audit report template into a real report?

Yes, as an original drafting aid, but replace every illustrative system detail, count, criterion, finding and decision with verified engagement information. Preserve the distinction between facts, hypotheses, management plans and limitations. Do not leave the specimen’s outcome in place merely because the format is convenient. The final report must be reviewed against the actual scope, evidence and reporting requirements.

Does the filled specimen represent an OpenForAI client audit?

No. The procurement tender-addendum case, identifiers, configurations and results are hypothetical teaching material. They are not claims about a real customer, completed engagement or observed performance. A real report requires actual evidence, agreed criteria, responsible reviewers and authorised issue. The specimen is designed to show how fields can be completed without implying certification or an unrestricted assurance conclusion.

Should the report template include a severity rating for every finding?

Include a rating where the engagement uses an agreed framework, with the rationale and evidence limitations. Do not copy a rating from a specimen or assign one simply to fill a field. Explain consequence, exposure, existing controls and uncertainty. If evidence is insufficient for a defensible rating, record that state and the decision needed rather than implying that an unrated issue is harmless.

How should sample counts be presented in the report?

State the population or case set, selection method, unit, exclusions and relevant denominators. Distinguish purposeful challenge cases from probability samples and unique cases from repeated runs. The specimen’s 12 packs are illustrative purposeful selections and do not estimate prevalence. Use actual counts and a suitable statistical method where inference is intended; never retain example numbers as though they were audit results.

Can the template be used for an Arabic AI audit report?

Yes, with a complete Arabic report that preserves the same scope, criteria, numbers, evidence references, findings and limitations. Use competent review for source-language meaning and professional terminology. Do not turn the Arabic version into a shortened marketing summary. Where the underlying evidence is translated, identify the derivative and any unresolved ambiguity that affects the conclusion.

What should be written when management has not agreed an action date?

State the response status honestly and identify the accountable decision owner and outstanding commitment. Do not invent a deadline or present a draft suggestion as an agreed action. Explain any interim conditions and the escalation route. Once management provides an authorised response, record it through the controlled report or findings-register process while preserving the earlier status and timing.

Should raw evidence be embedded in the AI audit report?

Usually include controlled references and only the excerpts necessary to understand the finding. Keep sensitive or voluminous support in an authorised repository with appropriate access. The report should remain traceable without unnecessarily distributing personal data, confidential documents or credentials. Record the evidence set and versions used so later review can reconstruct the basis of the conclusion.

Can this template issue a pass certificate after the findings are fixed?

No. The template is not a certification scheme or an authority to issue a formal assurance opinion. A retest can support a bounded judgement that agreed correction criteria were met within a defined scope. Certification or regulated assurance requires the appropriate separate engagement and competent authorised provider. Do not convert a technical report into a universal approval badge or guarantee of future system behaviour.
